The inspector’s account

Licensing Program Analyst (LPA), Angela Panushkina, conducted a CASE MANAGEMENT visit at this facility to issue deficiency in conjunction with complaint control #31-AS-20260105115721 and met with the Director of Hospitality. LPA also attempted to contact the CEO, Zane Schulte, to explain the reason for the visit. LPA left a voicemail with a call back number.

At approximately 10:20am, LPA conducted a physical plant walk through, to ensure that the facility is in compliance with rules and regulations. LPA observed that the garage was converted into an office (for the staff members) and the room previously used as an office was converted into a bedroom for Client #1 (C1). LPA reviewed facility’s e-files and observed the following:

During the pre-licensing visit conducted on 03/19/2024, LPA Panushkina was informed that five (5) bedrooms will be designated for client use. One (1) bedroom will be shared room, and the other four (4) bedrooms will be private. Additionally, LPA conducted review of the Facility Sketch (Floor Plan/LIC999) along with the Fire Safety Inspection Request STD850 and observed that the fire clearance (inspection date 12/26/2023) was granted only five (5) bedrooms, designated for clients use. LPA did not observe new request for the fire clearance to convert the staff office into bedroom #6 and/or the garage into the office.
