Facility Evaluation Report

At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. LPA met with the Administrator arrived shortly after, and LPA explained the reason for the visit.

A tour of the physical plant was initiated at 10:00am and the following was observed:

Kitchen: The kitchen area is equipped with a refrigerator, microwave oven and sink. There were adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of six (6). Fire extinguisher was last serviced on 10/09/2024.

Bedrooms: There are four (4) private and one (1) shared bedroom designated for clients use. All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space.

Bathrooms: Each client bedroom has its own bathroom. The bathroom in master bedroom #1 is the only client bathroom equipped with a bathtub. In addition to the five (5) bathrooms located in each client room, facility has two (2) additional community bathrooms that are available for client use. The hot water delivered in the client bathroom measured at 107.6°F.

Common Areas: The facility maintains a comfortable temperature of 70°F. LPA observed all common areas furnished with adequate furniture to accommodate a maximum capacity of six (6) clients.
