On 08/02/2024 at around 11:40 AM Licensing Program Analyst (LPA) Socorro Leandro arrived to the facility mentioned above. LPA met with Vice President Mary Artman and explained the purpose of the visit.
LPA delivered an amendment to a deficiency cited on 08/01/2024.
Deficiency regarding Acceptance and Retention Limitations for Client 1.
Client 1 was diagnosed with Alzheimer & Dementia in 2023 and then again in 2024.
An exit interview was conducted, and a copy of this report and deficiency was left with the Vice President along with their Appeal Rights.
