Facility Evaluation Report
On 08/22/2025 Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced annual required visit using the CARE Inspection Tool. LPA spoke with Administrator Richard Flowers via phone and the purpose of this visit was discussed. LPA Calderon met with family member of Administrator Erica Alexander who assisted with the visit. The facility is licensed to operate for (4) ambulatory clients ages 18 to 59 years. Currently the facility does not have any clients.
The facility is a one family home located in a residential neighborhood. The property consists of the following: 3 client bedrooms, 2 common bathrooms, staff office, living room, kitchen, dining room, attached garage which houses the washer and dryer and an outdoor shaded area.
LPA Calderon and Erica Alexandra toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of the visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. The shower was free of mold/mildew, there was adequate lighting, and sufficient toiletries were accessible to clients. The water temperature properly measured between 110F
