Facility Evaluation Report
On 02/24/2026, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Jennifer Criddle as the purpose of visit was explained. LPA was later joined by DSP Johnathan Bartolome who would assist with completing the visit. The facility is licensed to operate for (4) developmentally disabled or Mentally Ill adults ages 18 through 59. Approved for (4) ambulatory only. Clients are linked to the Harbor Regional Center. Facility fees are current, there is an active liability insurance (BSICDMRC0107104625RE Exp: 05/31/26)
The facility is a single-story structure located in a residential neighborhood. The home consists of (2) client bedrooms, (3) bathrooms, (1) staff room, a living room, dining-room, a kitchen, a staff work area with an additional sitting area and TV, and a patio. LPA found that the mattresses and box springs were in good condition, there was adequate lighting, and plenty of dresser and closet space. The facility was observed to be clean, sanitary, and in good repair. Additionally, bed linens, comforters, and bath towels were adequately stocked during the visit. The bathrooms were found to be in compliance with Title 22 regulations, the toilets and water faucets worked properly. The shower was free of mold and mildew, and there was adequate lighting and sufficient toiletries accessible to clients. The water temperature measured at 114.8°F.. A supply of perishable and non-perishable food supply was adequately stocked. The carbon monoxide/smoke detector combo was operational, and fire extinguishers were fully charged. The last facility disaster drill was on 02/03/26. Toxins and knives were securely locked away from clients. Medications were centrally stored and properly locked. A first aid kit and manual were observed.
