Facility Evaluation Report
On 01/11/24, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Martha A. Flores as the purpose of the visit was explained. The facility is licensed to serve four (4) non- ambulatory clients ages 18-59. The current facility census is (4), and clients are linked to the Harbor Regional Center. Administrator provided with annual fee information, facility has an active surety bond.
The facility is a single-story structure located in a residential neighborhood and consists of the following: 4 bedrooms, 2 common restrooms, laundry area, dining area, kitchen, pantry, linen closet, 1 staff office, a designated outdoor shaded area and a detached garage that stores additional hygiene products, extra PPE and extra linen. The facility has a working landline, 1 phone in the dining area, 1 phone in the kitchen and 1 phone in the staff office. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to clients, emergency food supply observed. There are no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards.
Client bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F..
LPA conducted a records review of 2 staff records, 2 client records, 2 medication administration records, and 2 P&I ledgers, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 12/21/23 and disaster drill conducted on 12/19/23, 1 fire extinguisher fully charged and mounted in the kitchen, carbon monoxide and smoke detectors are operational, internet service observed.
During today’s visit no discrepancies were observed. Exit interview conducted with Administrator
