Facility Evaluation Report
On July 16, 2025, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced annual required visit using the CARE Inspection Tool. LPA was met by Anchie Reyes Administrator and the purpose of today's visit was explained.
The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for two (2) hospice residents
The facility is a single-story structure located in a residential neighborhood. It consists of the following: 5 resident bedrooms, 2 common bathrooms, living room, dining room, kitchen, detached garage and outside shaded patio area with table and chairs.
LPA inspected all 5 bedrooms; the mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked.
