Complaint Investigation Report
Based on interviews, there is insufficient evidence to support the allegation: Staff hit resident with an object. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
Allegation #2: Staff spoke to resident in an inappropriate manner.
On 08/30/23, at 11:00am-01:30pm, LPA interviewed S1-S5. 4 of 5 staff denied the allegation that Staff spoke to resident in an inappropriate manner. Most of the staff state that the staff never speaks to the clients in an inappropriate manner. LPA interviewed C1-C10. 9 of 10 clients denied that staff speaks to them in an inappropriate manner. Further, 9 of 10 clients state that they feel safe in the facility.

Allegation #3: Staff does not treat resident with dignity or respect.
On 08/30/23, at 11:00am-01:30pm, LPA interviewed S1-S5. 4 of 5 staff denied the allegation that Staff does not treat resident with dignity or respect. Most of the staff state that they treat the clients with dignity and respect. They state that they don’t yell, harass, threaten, or bully clients in any way that would disrespect them. LPA interviewed C1-C10. 9 of 10 clients stated that they are treated with dignity and respect and like living in the facility.
