Facility Evaluation Report

On June 19, 2025, Licensing Program Analyst (LPA) Ernand Dabuet carried out an unannounced case management visit at the facility to address specific health and safety concerns. Upon arrival, the facility's administrator, Jemimah Mejia, welcomed the LPA and was cooperative throughout the visit.

To begin the assessment, the LPA carefully reviewed the current Client Roster and Personnel Report, LIC 500. The review revealed that the facility employs a total of 20 staff members, ensuring staffing with a minimum of two caregivers scheduled for each of the three critical shifts: morning (AM), evening (PM), and night (NOC). Additionally, Mejia informed the LPA that medical professionals conduct weekly visits to the facility, focusing on educating clients about various substance abuse topics, which actively strengthens their awareness and understanding of essential issues.

Mejia provided an activity attendance log sheet for verification and monitoring purposes that included documented attendance for specific dates: May 3, 2025, May 17, 2025, and March 29, 2025. These logs contribute to the facility's accountability regarding client engagement in educational and recreational activities. The facility has a current administrator's certification for Jemimah Mejia #6060577735 Expiration: 07/01/26.

Following the documentation review, the LPA conducted a thorough physical tour of the facility. During this inspection, the LPA observed the environment and found no apparent health or safety issues, indicating that the facility is maintaining adequate standards for the welfare of its residents.

An exit interview was conducted with Jemimah Mejia, a copy of the report was provided.
