The inspector’s account

On 09/03/21, Licensing Program Analyst (LPA) Susan Campos conducted an unannounced annual required visit with a primary focus on Infection Control measures using the new CARE Inspection Tool. LPA was allowed entry into the facility by Rodolfo Dimatulac, Administrator. LPA met with licensee and administrator Rodolfo Dimatulac and explained the purpose of the visit. This facility is licensed to serve 76 Mentally Disabled Adults, with a current capacity of (69), all are Ambulatory and none have restricted healthcare conditions. There are (11) adults over the age of 59. The last fire/emergency disaster drill was completed on 7/15/21. The last fire inspection was conducted on 9/23/21. The Administrator certificate expires 7/9/22 The facility is located in an industrial area and is composed of two large buildings with 18 rooms in south building and 20 resident rooms: South Building (downstairs = room # 2,3,4,5,6,7,9, room #8- staff lounge/bathroom, common bathroom, shower room, activity room) upstairs (room #11,#12, #14, #15, #16, #17, #18, #19, #20,#22, #23, common area restroom, common shower. North Building (downstairs = rooms #24, #25, #26, #27, #28, food supply/pantry, laundry room, common area restroom, (2) dining rooms, commercial kitchen, activity room, storage room) Upstairs - (rooms #29, #30, #31, #32, #33, #34, #35, #36, #37, #38, #39, #41.) LPA toured the physical plant with Mr. Dimatulac, and also inspected the following client rooms (#3,#4, #5, #9, #25, #26, and #27), and also checked the hot water temperature. There were no bodies of water or obstructions on the premises. Beds and bedding supplies were in good condition, adequate lighting provided, storage for client personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured between (112-116) degrees Fahrenheit. A comfortable temperature of 73 degrees was maintained in the facility. LPA observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to clients. The kitchen was inspected and there is sufficient perishable and non-perishable food available maintained properly. A fire extinguisher was charged, smoke detectors and carbon monoxide were operable. Fire Drills were observed to be maintained in order and accurate.
