Facility Evaluation Report

On 6/06/2026 at approximately 10:15 AM, Licensing Program Analysts (LPA) Angelica Segovia conducted an announced Case Management visit to this facility to ensure the facility was non-operational and ready for closure. LPA met with the Administrator , Serozh Kirakosyan.

The reason for today's visit is to document the closure of the facility which was initiated by the Licensee. On 5/15/2026, LPA received notice of intent of closure of the facility. The Licensee verbally surrendered the facility's license to Community Care Licensing Division (CCLD) on 06/06/2026. The facility since becoming licensed on 4/11/2024 remained non-operational with zero (0) residents.

During today's visit, LPA conducted a physical plant tour with the Administrator and observed there to be no residents living in the facility. LPA observed the facility to be vacant. LPA’s interview with the Administrator confirmed they voluntarily surrendered their license to finalize the closure of the facility. This closure will take effect today 6/06/2026.
