Licensing Program Analyst Abeye Duguma (LPA) conducted an unannounced annual visit. LPA attempted to gain access to the property but to no avail. No activity on the property. LPA spoke with licensee by phone. Licensee stated they no longer wish to maintain a license. LPA advised licensee to contact the assigned LPA to schedule a closure visit.
