Facility Evaluation Report

At approximately 9:20 a.m., Licensing Program Analyst (LPA) Evelin Rios conducted a Case Management – Other visit to confirm the closure of the facility. LPA knocked on the door and nobody answered. LPA contacted the licensee representative Heaven Roberson by telephone and left a voicemail informing her LPA was at the facility to confirm the facility is not operating. LPA Rios also sent an email.

On 09/30/2025, LPA Lorena Casillas attempted to conduct an unannounced annual inspection. On 11/21/2025, the licensee representative informed LPA Rios by telephone that no clients had been admitted to the facility and that they were considering forfeiting the license. On 12/03/2025, LPA Rios attempted another unannounced annual visit. After contacting the licensee representative, LPA Rios was informed that there were no clients in care and that the home was currently occupied by a tenant.

On 12/11/2025, Community Care Licensing (CCL) emailed the facility regarding overdue annual fees. On 01/25/2026, LPA Rios mailed a 14 day notice advising the licensee that the facility was under consideration for closure due to the overdue annual inspection, which could not be completed. On 02/11/2026, the licensee representative confirmed they were not operating and wished to close the facility. During today’s visit, Licensing Program Manager (LPM) Mary Flores contacted the licensee representative and confirmed the facility will forfeit license. LPA Rios knocked on two neighboring homes, but no one answered.

The facility’s effective closure date is 02/23/2026. LPA will e-mail the closure survey to the email on file. LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office.
