On 12/04/2025 at approximately 2:30 PM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the Program Director, Leticia Campbell and stated the reason for their visit.
LPA requested census, staff and client roster.
Due to time constraints, the annual inspection will be done at a later date.
There were no immediate health and safety hazards observed during the day of inspection. Exit interview conducted and a copy of this report was provided to the Program Director.
