Facility Evaluation Report

Licensing Program Analyst (LPA) Evelin Rios in conjunction to a complaint investigation control number 31-AS-20260514092010 , LPA conducted an unannounced case management – Deficiencies visit to the facility. LPA met with the Program Director, Marine Guloyan and explained the reason for the visit.

LPA interviews with the Program Director, one (1) staff member, and three (3) clients revealed that the facility monitors clients’ phone calls. Clients reported that staff sit across from them while they are on the phone. The Program Director stated that facility telephones are available for emergencies and that clients may request a private call. However, interviews with clients indicate they were not aware that requesting a private or confidential phone call was an option, and some did not know it was their right to do so.
