Facility Evaluation Report
On 07/28/26 Licensing Program Analyst (LPA) Lorena Casillas conducted an unannounced case management visit. LPA met with the Program Manager (PM) Lorraine McKesson and disclosed the reason for the visit.
Today’s case management visit is to follow up on an unusual incident previously reported on 07/10/26 via SOC341 in which Client #1 (C1) was allegedly sexually assaulted by Staff #1 (S1). The purpose of the visit is to obtain additional information regarding this incident. Between 09:30am and 11:30 am LPA interviewed PM and two (2) staff.It was reported that on 07/10/26 an adult day program trainer, S1, grabbed C1’s right buttocks and grabbed C1 by the waist. This incident was not reported to Community Care Licensing (CCL) within the reporting time frames indicated in Tittle 22, instead it was reported by a different entity. As of today’s date, 07/28/26, LPA has not received an incident report. LPA interviewed PM who stated that C1 does not attend Project Independence but instead is next door at Desert Haven, a facility not overseen by CCL. PM also informed LPA that S1 has not worked at this facility since June 2026 and instead works next door at Desert Haven. LPA interviewed Human Resource (HR) Generalist, and they confirmed that C1 is not enrolled at this facility. HR further confirmed that S1 does not work at this facility and has not worked here since June 2026. LPA interviewed Finance Director who confirmed the information provided to LPA by HR and added that an investigation has already taken place with Desert Haven where the outcome was unsubstantiated. CCL does not oversee Desert Haven however, the incident has already been investigated and concluded. LPA reviewed records and was able to discover that S1 is not associated with this facility. LPA reviewed client roster and observed that C1 is not enrolled in this day program. Therefore, this case management visit concludes that the incident was not reported to CCL because it did not take place at this facility.
