Facility Evaluation Report
On 12/08/2025 at 09:20 am Licensing Program Analyst (LPA) Lorena Casillas arrived at the facility for an unannounced one (1) year Required visit. LPA was greeted and granted access by a staff member. LPA met with Administrator Amalia Sandoval. LPA informed Administrator of the purpose of the visit. Entrance interview conducted.
INFECTION CONTROL and EMERGENCY PREPAREDNESS PLAN: LPA reviewed facility Infection Control Plan and Emergency Preparedness Plan to make sure facility is following protocols.
A tour of the physical plant was conducted with Administrator at 9:45 am. The facility has four (4) bedrooms and three (3) bathrooms currently occupying four (4) clients. A fire clearance was approved for four (4) ambulatory clients for a total capacity of four (4). One (1) client was present, and two (2) clients were attending program.
KITCHEN: At 9:50 am LPA toured the kitchen. The facility has a Kitchen area that is equipped with a refrigerator, microwave oven and sink. There is an adequate supply of two (2) days perishable and a seven (7) day supply of nonperishable foods and dining ware to accommodate a maximum capacity of four (4). Knives were locked under the kitchen sink within a locked box. Medication is locked in a kitchen cabinet. The facility has one fire extinguisher that was fully charged on 08/25/2025 located in the kitchen. There is a first aid kit in a kitchen cabinet, and it is equipped with the necessary supplies.
