Facility Evaluation Report
On 02/02/2026 at 09:45 am, Licensing Program Analyst (LPA) Lorena Casillas conducted an unannounced required yearly inspection. There was no answer at the door and LPA contacted Licensee over the phone. Licensee stated that there were no clients currently occupying the facility and that it is vacant, however Licensee would meet with LPA. Licensee Meline Atamian met with LPA shortly after. LPA explained the reason for the visit and an entrance interview was conducted.
Infection Control and Emergency and Disaster Plan: LPA reviewed facility Infection Control Plan and Emergency and Disaster Plan to make sure facility is following protocols.
A tour of the physical plant was conducted at 11:45 am with Licensee. The facility has four (4) bedrooms and two (2) bathrooms. It is currently not occupied with any clients. The tool kit was not used during this visit as there are no clients.
Kitchen: The facility has a kitchen area that is equipped with a refrigerator, microwave oven, dishwasher and sink. There was an adequate supply of nonperishable food and dining ware to accommodate a maximum capacity of four (4). Knives are locked in a kitchen cabinet and are within a locked box. There is a fire extinguisher located in the kitchen and last serviced on 12/20/25. Licensee will purchase perishable foods once there are clients in the facility.
