Facility Evaluation Report
Licensing Program Analyst (LPA) Evelin Rios conducted an unannounced Case Management Incident visit to the above facility. The purpose of this visit is to address information reported by the facility on a Special Incident Report (SIR) submitted to the Licensing related to a warrant served for a search and seizure of Client #1’s (C1’s) personal property. LPA also checked to ensure the health and safety of all the clients in the facility.
Prior to visit LPA discussed the information on the SIR with the administrator via telephone. According to the administrator the facility's director visited the facility and discussed internet safety with all the clients. During the visit, LPA met the administrator designee, Leslie Clarkson, and C1. LPA obtained contact information for the three (3) clients who were not currently at facility due to being at work or day program. LPA also conducted a tour of the physical plant to ensure the health and safety of the residents in care. No issues observed.
According to the report the incident occurred on 4/02/25 at approximately 6:00 a.m., staff #1 (S1) and clients were visited by FBI officers asking everyone residing in the home to come outside. S1 and all four (4) of the clients went outside and were placed in handcuffs. Once inside the FBI realized the home was a licensed Adult Residential Facility (ARF) and removed S1’s handcuffs. S1 was asked to show them around the facility. The FBI then informed S1 the purpose of the visit was to issue a court warrant on C1 for allegedly being involved in illegal activities online with minors. The FBI seized C1’s personal property and provided two warrant court documents and contact information. The FBI informed staff further investigation is needed. C1 is still in the facility.
