Facility Evaluation Report

On 02/09/2026 Licensing Program Analyst (LPA) Lorena Casillas arrived at facility above to conduct a subsequent visit for complaint # 31-AS-20250902173352. This Case Management is not directly related to the original complaint visit, however deficiencies were discovered during the investigation. LPA met with Administrator Anthony Powell and explained the reason for the visit.

The complaint investigation was conducted by Jose Santana, Investigator from Community Care Licensing Division of Investigations Branch (IB) and it was discovered that on 09/23/25 an eviction notice was issued for Client #1 (C1). During the interviews conducted by IB, it was discovered that the previous Administrator Vashan Bobney issued a 30-day eviction notice for C1 after being instructed by Licensee/Administrator Anthony Powell to do so. Although North Los Angeles Regional Center (NLARC) and Department of Child Protective Services (DCFS) were notified, Community Care Licensing (CCL) was not. LPA Casillas reviewed special incident reports (SIR), emails and text messages submitted to CCL from 09/01/25 to 11/01/25 and could not locate notification of the eviction. Furthermore, a copy of the eviction notice was provided to LPA Casillas by IB investigator and LPA discovered that the notice was lacking elements necessary to be included in an eviction notice per Tittle 22 regulations. Therefore, citations for failure to notify CCL of an eviction and for issuing an eviction notice that lacked necessary information, will be issued.
