Facility Evaluation Report

On 12/01/25 Licensing Program Analyst (LPA) Lorena Casillas met with Administrator Priscilla Isordia for a case management visit. The purpose of the case management visit is to address a deficiency observed during the course of investigation for complaint # 31-AS-20251121104847. The deficiency was not alleged but is related to the complaint.

On 10/30/25 staff found there to be bed bugs in the facility. Although facility was fumigated by an exterminator on 10/30/25, 11/05/25 and 11/11/25 and clients were temporarily relocated, the incident was not reported to Community Care Licensing (CCL) as per regulations, nor was LPA made aware of the situation. LPA informed Administrator that any unusual incidents that threaten the physical, emotional health or safety of any client needs to be reported to CCL within the time frames outlined in Tittle 22. Therefore, due to not reporting the incident of bed bugs and the relocation of clients, a citation will be issued.
