Facility Evaluation Report
Licensing Program Analyst (LPA) Lorena Casillas met with Administrator Priscilla Isordia for an unannounced one (1) year Required visit for this facility. LPA arrived at 10:00 am and was greeted and granted access by staff. Staff called Administrator, Priscilla Isordia and LPA explained the reason for the visit. The Administrator arrived shortly after. Entrance Interview conducted.
One (1) client observed in the living room, two (2) clients in their bedrooms, and the other client was at day program.
A tour of the physical plant was conducted with Administrator at 11:40 am. The facility has four (4) bedrooms and two (2) bathrooms currently occupying four (4) clients. The facility is Fire Cleared for four (4) ambulatory clients. There is awake staff at night.
Infection control: LPA reviewed facility infection control plan (approved on 08/18/22) to make sure licensee was following current infection control recommendations.
Kitchen : LPA conducted a tour of the kitchen at approximately 11:40 am and observed there to be sufficient stock of two-day perishable and seven-day non-perishable foods. Frozen foods are properly wrapped and stored. Food storage and preparation areas are clean and clear of clutter. LPA observed all knives, sharp object, and medications locked and inaccessible to clients in care. There is one (1) fire extinguisher located in the kitchen. The Fire extinguisher was observed to be fully charged and last serviced 06/18/24.
