Facility Evaluation Report
Licensing Program Analysts (LPAs) Lorena Casillas and Melissa Spaeth met with Administrator Marie Bru for an unannounced one (1) year Required visit for this facility.
LPAs arrived at 10:15 am and there was no one at the door. LPA Casillas called Administrator and she said that she would be at facility in approximately 45 minutes. Administrator showed up at 11:30 am. LPAs informed the Administrator of the purpose of the visit.
Infection control: LPAs reviewed facility mitigation plan (approved on 10/17/2022) to make sure licensee was following current infection control recommendations.
A tour of the physical plant was conducted with Administrator at 12:12 pm. The facility has four (4) bedrooms and two (2) bathrooms currently occupying four (4) clients. One (1) bathroom and bedroom is designated for staff use only. The facility is Fire Cleared for six (6) ambulatory.
LPAs observed the living room to be neat and clean along with the dining room. The facility maintains a comfortable temperature at 73°F. The smoke detectors were tested and observed to be operational at 12:20 pm. There are three (3) fire extinguishers, one (1) is located in the kitchen and two (2) are located in the hallway on each side of the house. The Fire extinguishers were observed to be fully charged and last serviced on 05/02/23.
