Facility Evaluation Report
On 02/23/2026 at 10:00 am Licensing Program Analyst (LPA) Lorena Casillas arrived at the facility for an unannounced one (1) year Required visit. LPA was greeted and granted access by staff. Administrator was contacted, and LPA explained the reason for the visit. Administrator explained that they would try to meet with LPA, in the interim they designated staff member Yasmine Zachery to assist LPA and if needed, sign the report. An entrance interview was conducted with designee. Administrator was able to join LPA later during the visit.
Infection Control and Emergency Preparedness Plan: LPA reviewed facility Infection Control Plan and Emergency Preparedness Plan to make sure protocols are being reviewed and/or updated. Plans were discussed and updated.
A tour of the physical plant was conducted with the designee at 11:30 am. The facility has four (4) bedrooms and two (2) bathrooms. It is currently occupied by two (2) clients. Facility is fire cleared for four (4) ambulatory clients.
Living and Dining: LPA observed the living room furniture to be clean and in good repair. LPA observed the dining area to be clean. The facility maintains a comfortable temperature at 70˚F. The smoke alarm and carbon monoxide detector were tested and were operational. The fire extinguisher is full and last serviced on 07/23/25. Files were observed to be in a locked cabinet located in the dining room.
