Facility Evaluation Report
Licensing Program Analyst (LPA) Galarza conducted a Case Management -Deficiencies visit due to observation made while investigating complaint control #: 28-AS-20250401101011 visit. The purpose of the visit was explained to Assistant Administrator Ann Hyde.
There are two (2) clients (C1 & C2) over the age of 59. They are ages 65 & 60. No age exceptions are on file. This poses a potential health and safety risk to persons in care.
Per Title 22, If acceptance or retention of an individual 60 years of age or older would result in the number of persons 60 years of age or older exceeding 50 percent of the census in facilities with a capacity of six or fewer clients ...... the licensee must request an exception in order to accept or retain the individual.
Client (C1's) bedroom walls are in disrepair i.e. dirty and with incomplete plaster/painting repairs.
Administrator on record is not current. Renee Davenport is the current Administrator.
Per C alifornia Code of Regulations, Title 22 deficiencies are cited.
Exit interview held with Assistant Administrator Ann Hyde. A copy of the report and appeal rights were provided.
