Facility Evaluation Report

An informal conference was held on 5/6/26 at the Monterey Park Adult and Senior Care Program Regional Office. Participants in the meeting were Fernando Fierros (Licensing Program Manager), Elena Mallett and Cynthia Chan (Licensing Program Analysts), and Percy Palada (Licensee/Administrator).

The purpose of this conference is to discuss the issues of non-compliance for the past three years. The following regulations were discussed:

· 87755 Inspection Authority of the Licensing Agency
