Facility Evaluation Report
On 03/17/26, at 11:29am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, annual visit. Matthew De Vera, Administrator was advised of the visit and toured with LPA.
LPA asked for the census, client, and staff files.
The facility has been licensed as a Adult Residential Facility. All three (3) clients are ambulatory. The physical plant was toured inside and outside at 12:45pm. It is a single, story home.
Kitchen area was sufficiently stocked with seven (7) days of perishable and seven (7) days of non-perishable food. There is one (1) refrigerator in the kitchen area. The cabinets have canned goods. Sharps/Knives are kept secured and locked and are on the kitchen counter. The medication is kept in the kitchen area locked and secured in a pantry area. There are toxins that are kept under the sink locked and inaccessible to clients. There is a fire extinguisher fully charged and dated 01/2026.
There is one (1) washer and dryer locked in a room leading towards the garage. There is another fire extinguisher fully charged and dated 01/2026 by the entrance of the garage.
