Facility Evaluation Report

Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced Case Management visit with LPA Bunker met staff member Frank Jacque and explained the purpose of today's Case Management visit. There is currently two (2) Westside Regional Center (WRC) Adult Residential Care Facility (ARF) consumer in placement.

During the visit, LPA Bunker spoke with Administrator Lawrence Phillips via telephone. Mr. Phillips assured LPA Bunker that the individual named in the decision and order is not employed at the facility, has never worked for the facility, and has been disassociated. LPA verified that the named individual is not a staff member at WCAY, Inc. and was not present at the facility.

Mr. Jacque stated that he has been working at the facility since 2006 and has never heard of the individual in question. He further confirmed that no person by that name has ever worked at their facility or at any WCAY, Inc. location.
