Facility Evaluation Report
On 07/02/2026, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Required 1-Year Annual Visit using the full CARE Inspection Tool. LPA met with Associate Executive Director, Olivia Pena, and the purpose of today’s visit was explained. LPA was granted entry into the facility.
The facility is and Adult Day Program, licensed to serve a hundred (100) developmentally disabled Clients ages 18 and above. Twenty (20) of the clients may be non-ambulatory. The program is vendorized by the Westside Regional Center and South Central Los Angeles Regional Center. The facility has a ratio of 1 to 3. Physical Plant/Structure The facility is a single-story structure located in a commercial area. The business operates from 9am to 2 pm, five (5) days a week, Monday through Friday. The facility consists of a gated parking lot, reception area, lobby, changing room, art and craft room, staff training room, restrooms, First Aid/Health Safety Infection Preventionist Office, kitchen, six (6) administrative offices, executive director’s office, transportation office, client locker area, dining area, storage room, staff break room, patio, and outside shaded area. LPA did not observe any bodies of water on the premises.
Program During the facility tour, LPA observed the facility was appropriately furnished during time of visit. LPA observed an ample supply of material and supplies LPA observed games, activities, crafts and materials available for client use. LPA observed all walkways inside and outside the facility were observed clean, clear, and free of obstruction, debris, and hazards. The programming schedules are developed by the Director, Support Staff, and Clients and is posted at the dining room entrance. The day program has current activity schedules, and each instructor follows monthly plans.
Files LPA reviewed the files for five Clients and observed they have the required documents and consents. LPA reviewed the files for five staff and observed they have the required clearance, certification, training, and documents. During file review, LPA observed the licensing fees are current.
