Facility Evaluation Report
On 07/15/2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced annual inspection visit. LPA introduced herself to Myrna and Rosa support staff who were informed of the purpose of the visit LPA was allowed entry into the facility. Emyflair Dojillo Program Director arrived around 11:00 AM, and she was informed of the purpose of the visit. The Administrator Reynaldo Aro, arrived around 11:30 AM and he was informed of the purpose of the visit.
The facility is an Adult Day Program and is licensed to serve 45 Developmentally Disabled Adults. At the time of visit there were 33 clients attending program 14 clients were present and 19 were in the community. The facility consists of two buildings. Building #1 contains two staff offices, an activity room, an isolation room, kitchen and two bathrooms. Building #2 contains a large activity room, two bathrooms and a storage area.
At 10:45 AM, LPA and Rosa Castaneda support staff toured the facility inside and outside and there were no obstructions. LPA observed outside 3 shaded areas with large rectangular tables and chairs. Two of the shaded areas are for activities to be done outside. There is an outdoor activity area that can be used for basketball/volleyball or other games. There is a vegetable garden that is maintained by clients with the assistance of staff. LPA Allen Also observed the weekly activity schedules.
At 11:00 AM, LPA Allen reviewed four (4) clients files for admission agreements, updated physician reports, needs and services plans, all of which all appeared to be current.
LPA also reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which all appeared to be current.
