Facility Evaluation Report

On 09/10/2025, Licensing Program Analyst (LPA) Jose Calderon conducted a Case Management visit at this facility. LPA met with designated administrator Francis Liwanag who allowed entry in this facility. LPA Calderon informed Administrator Liwanag the purpose of the visit is to conduct a records review.

Investigation revealed the following: LPA conducted a tour of the entire facility. During the inspection, LPA observed the following: (4) bedrooms (2) bathroom, kitchen, living room, den, patio, and garage LPA observed all (6) residents residing at this home requiring assistance with assisted daily living (ADLs) This property address is licensed to Ace Elderly Homes formally Santa Fe Home III #198602162. LPA Calderon requested a copy of the LIC500/Guardian Employee Roster and checked 6 residents and 4 staff records.

No deficiencies cited during today's visit.

An exit interview was conducted with Administrator Francis Liwanag, and a copy of the report was provided.
