Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by staff and the reason for the visit was explained. Licensee Rowena Santos arrived shortly after to assist.
The facility is licensed to serve age range 60 and over, six (6) non-ambulatory of which one (1) may be bedridden and an approved hospice waiver for six (6). Bedroom #3 is for the bedridden resident. There are currently two (2) residents receiving hospice services.
The facility is a single-story home located in a residential area of Glendora. The home consists of a kitchen, living room, dining area, two (2) bathrooms, five (5) resident bedrooms, an attached garage with laundry, front and backyards.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
The front and backyard are well maintained and there are no pools or large bodies of water. There is a covered patio providing a shaded seating area located in the backyard with sufficient seating for residents in care. Passageways and exits are free of obstructions.
Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to residents, were observed to be inaccessible to residents.
