Facility Evaluation Report

On 4/24/2023, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Laura Ramirez/Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (6) (developmentally disabled or Mentally Ill) adults ages 18 through 59. Currently, the home has (3) clients. The clients are (Westside Regional Center clients. 1 client have Restricted Health Care Conditions (Nebulizing Machine for Asthma), and none are utilizing postural supports or protective devices.

The facility is a single-story family home located in a residential neighborhood. The facility consisted of the following: Living room, dining area, office, kitchen, 4 bedrooms, 2 bathrooms, office, activities room, laundry area, attached garage, shaded area, and indoor/outdoor activity areas. Bedrooms #1 thru #4 are designated as the client's bedrooms.

LPA conducted a records review of (3) client records, (5) staff records. All clients & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (3) Client Medication Administration Records (MAR) and did not observe any discrepancies at the time of visit.
