Licensing Program Analyst (LPA) Tihesha Smith conducted a case management visit in conjunction with a complaint visit:31-AS-20260410162348. LPA review of LIC 500 Personnel report has new staff at facility. Discussion with lead staff reveal the facility had a change in ownership. LPA Smith unable to locate any documents that the Regional department was contacted prior to the change.
