Facility Evaluation Report
On 07/18/2025, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with House Manager Deborah Lewis. LPA explained the purpose of the visit and were accompanied by House Manager inside and outside the facility during this inspection.
The facility is a single-story structure located in a residential neighborhood. It consists of the following: 4 bedrooms, 2 bathrooms, family room, living room, kitchen, dining room/office, indoor and outdoor activity area, and laundry room in the detached garage. The facility is licensed to operate for (4) developmentally disabled or Mentally Ill adults ages 18 through 59. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. Client’s bedrooms were checked. Mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Bathroom toilets and water faucets worked properly. Adequate lighting and toiletries accessible to clients. LPA Richard tested hot water temperature and it measured 114.2F degrees Fahrenheit.
