Facility Evaluation Report

Licensing Program Analyst (LPAs) Luis DeLeon, Mayra Cota and Licensing Program Manager (LPM) Adeline Ho conducted a Case Management Visit-Deficiencies on January 16, 2025. LPAs and LPM met with Leonard Turner and explained the reason for visit. The purpose of the visit is to follow on items from previous visit.

During visit, LPAs reviewed staff roster, and conducted water temperature check. The water temperature on the bathrooms had the following readings: South Wing : Bathroom 1: 100.1, Bathroom 2: 108.6, and Bathroom 3:112.5 North Wing: Bathroom 1:108.3, Bathroom 2: 107.6, and Bathroom 3: 102.2

The LPAs observed the installation of a new water heater on the north wing, currently set at lowest setting. The bathroom farthest from the water heater is not reaching minimum regulation temperature. Licensee stated that setting was going to be adjusted to reach the desired temperature and a log will be maintained to monitor the ideal setting to comply with water temperature for all three (3) bathrooms.

The licensee stated that he will follow the recommendation of contractor to install individual water heater under south wing bathroom sink (South wing bathroom 1) which will resolve the issue with the bathrooms farther away from the main water heater. Licensee will send picture of new water heater installed under the sink.

Licensee is currently working with Guardian to obtain a temporary password to access his Guardian account. Licensee will inform DSS with Guardian account status. LPAs obtained LIC 500 and LIC 9182. LPAs will assist licensee with staff association while Guardian account login information is sent to licensee.

Licensee will send the infection control plan to department by end of week.

Exit interview held, and a copy of this report was given to licensee.
