Facility Evaluation Report
Licensing Program Analyst (LPA) Alma Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Leonard Turner, and the purpose of today’s visit was explained. LPA was granted access to the facility.
The facility is an Adult Day Program (ADP) licensed to serve 100 ambulatory and 5 non-ambulatory consumers ages 18 and over. The facility is a one story building with 2 wings. The right side of the wing include lobby area, five offices, nursing station, quiet room, 2 storage rooms, staff lounge, isolation room with a restroom and 3 class rooms and a staff restroom. The left side of the wing, includes 3 consumer classrooms, kitchen area, 2 storage rooms, men's restroom and 2 unisex restrooms. There is also an outside patio in between 2 wings.
LPA and Administrator Leonard Turner toured the physical plant. All rooms were inspected. The facility walls/ floors were in good condition, and there is adequate lighting throughout the facility. The kitchen and dining room are clean and well maintained. The bathrooms were found to be within Title 22 regulation. A comfortable temperature for clients is maintained at all times. Hot water temperature measured between 105 degrees F and 120 degrees F (105.9F). The kitchen was inspected, and sufficient non-perishable food was adequately maintained. The facility does not provide lunch and only provides snacks. Clients bring their lunch to the program. Facility has storage area/shelves for the consumers' personal belongings. The fire extinguishers were properly charged, smoke and carbon monoxide detectors are operational. The first aid kit was stocked with manual and available for use. Exits/ Walkways around the facility were free of debris and hazards. The facility is in good repair. Toxins and sharps were locked and inaccessible to clients. Outside grounds were toured and no bodies of water were observed. The outdoor courtyard was equipped with a shaded seating area that is accessible for consumers use.
