The inspector’s account

Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Assistant Administrator Rodel Serrano and explained reason for visit. Administrator Jafer Cruz arrived shortly. The facility is licensed to serve 4 non-ambulatory clients ages 18 to 59 years old. The facility currently has 4 Clients serviced through South Central Los Angeles Regional Center. The facility is a single-story home located in a residential area in Downey, Ca. A tour of the facility includes: (4) client bedrooms, (2) bathrooms, linen closet, staff office, living room, dining area, kitchen, attached garage with laundry and storage, front yard and back yard and tool shed for storage.

LPA toured the facility and observed the following: Each client bedroom has the required furniture and bedding. There are extra clean linen and towels in hallway closet. Smoke detectors and carbon monoxide detectors were observed in each room and throughout the facility and are properly operating. The facility has one (1) fully charged fire extinguisher that is kept in the living room. Cleaning supplies, toxic substances, and sharps are inaccessible to clients in locked cabinets in kitchen. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 45 degrees F. LPA observed a sufficient supply of 2 days perishable food, and seven (7) days of non-perishable foods. There is an extra refrigerator in garage with more food. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction.

Four (4) staff files were reviewed and included criminal clearance record, CPR/ First Aid training, and health screening with TB. Four (4) client files were reviewed and included physical with TB test, and individual program plan (IPP). Last fire/earthquake drill was conducted on 05/05/2026. Infectious control plan and Disaster Plan 610 D were both reviewed. Four (4) client medications were reviewed and no errors were found. Medications are centrally stored and locked MAR log is used.

Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during today’s visit. Exit interview was held and a copy of the report was emailed to Rodel Serrano.
