Facility Evaluation Report

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection. LPA was greeted by staff and the reason for the visit was explained. Manager Beatriz Milian arrived shortly after to assist with the visit. The facility is licensed to serve 4 developmentally disabled and ambulatory only adults ages 18-59. The facility currently has 4 clients serviced by South Central Los Angeles Regional Center.

The facility is a two-story home located in Downey. Clients’ bedrooms are located on the first floor, licensee family residence is on the second floor. The first floor consists of three (3) client bedrooms, 2 ½ bathrooms, office room, dining room, kitchen, living room, family room, outdoor shaded patio areas, locked sheds in the backyard, outdoor washer and dryer, a detached garage/warehouse. There is a pool in the backyard with a net covering/fencing with a self-latching gate and outdoor jacuzzi with a locked cover.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: Facility has sufficient PPE supplies and the required Infection Control Plan.

Operational Requirements: Staff have proper training to meet the needs of the clients in care. Facility has an activity area furnished for outdoor use.
