Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Visit-Incident on 05/22/2025, stemming from incident report received on 05/11/2025. LPA was greeted by Direct Support Staff- Vena Miles and explained the purpose of the visit. Administrator Lashon Johnson arrived shortly after to assist with tour.
According to Unusual Incident/Injury Report received on 05/11/2025, staff self-reported that client#1 (C1) was observed to have a small bruise on their right eye on 05/09/2025, around 12pm. According to Administrator Johnson, C1’s primary care physician was contacted on 05/09/2025 & 05/12/2025 and left a voicemail requesting a call back. On 05/13/2025, Administrator Johnson called C1’s primary care physician for a third time and was instructed to take C1 to the nearest emergency room. C1 was treated and discharged back to their facility the same day with no new physician orders. All staff from previous shifts were interviewed and revealed no incidents occurred on their shifts with any clients.
On 05/11/2025, Downey Police arrived at the facility to interview Administrator Johnson, staff and view C1’s bedroom. According to Administrator Johnson, Officer S. Hinojosa, provided a business card with report# 25-175177 and exited the facility. On 05/22/2025, LPA Ramirez observed C1 seated in living room area. C1 appeared to be well groomed, and LPA Ramirez did not observe any noticeable injuries. C1 is non-verbal and limited comprehension, therefore was not interviewed. LPA Ramirez observed clients’ bedrooms to be clean and no hazards were observed.
No further action is required at this time. LPA Ramirez may return to gather additional documents. No deficiencies were cited during this visit. Exit interview was conducted. A copy of this report was provided via email.
