Facility Evaluation Report

Licensing Program Analyst (LPA) Glenn Trueman made an unannounced visit to the facility and was greeted by Staff Veronica Mendez and explained the reason for the visit.

The purpose of the visit is a Case Management visit to collect additional information regarding AWOL's by Client C1.

This is in reference to the initial complaint Control #28-AS-20250714152659.

At today's visit Staff S1- S3 all stated they do follow recommendations from the IPP for Client C1.

They also stated Client C1 last AWOL'd on 07/12/2025. Stated Special Incident Report (SIR) was submitted for 07/12/2025.

S1-S3 stated they did not tell law enforcement that they don't call because Client C1 AWOL's often. Also stated it is not true that their Administrator ignores their calls.

Said they do follow a set protocol regarding Client C1 AWOL'ing and the Administrator is contacted immediately for guidance and support.
