Facility Evaluation Report
Licensing Program Analyst (LPA) Val Maldonaldo and Tyler Reyes conducted an unannounced Required 1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPAs explained the purpose of the visit to the Program Director (PD) Tia Sommers of Mainstream Centers, INC. The facility is licensed as an Adult Day Care.
During today's visit, LPA Maldonado and Reyes conducted a tour of the physical plant with PD Sommers, reviewed (9) client files, (5) staff files and conducted interviews with (4) staff and attempted interviews with clients. LPA's also reviewed (3) client medications, which were centrally stored.
The facility is a single-story building operating as an Adult Day Care. It is licensed to serve (120) adults, ages (18) and over. There is a fire clearance approved for (120) where (24) can be non-ambulatory.
An approved Infection Control plan has been submitted to the department for review. The facility has an active and current auto insurance policy on file, as required for the facility vehicles.
(3) bathrooms- (1) was co-ed, (1) women's, and (1) men's with required grab bars
A hand wash station with multiple faucets, for client use
Multiple Carbon monoxide detectors observed throughout the facility- tested and operable
Program site observed to be clean, safe, sanitary and in good repair
Disinfectants/cleaning solutions/sharps are centrally stored and inaccessible
Water Temperature tested and within range at 105.8F
