Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection visit. LPA was greeted by Program Director Laura Bradshaw, and the purpose of the visit was explained.
The day program is licensed to serve a maximum of 45 non-ambulatory clients, ages 18 and over. Facility is operating within the scope of their license.
The facility includes three (3) activity rooms, relaxation room, office, changing room, kitchen, and three (3) client bathrooms.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
The facility is clean, safe, free of obstructions and in good repair. One tabletop was found to be loose and unstable. Technical violation was issued. Kitchen was observed clean and appliances were working properly. Food is not prepared at this program; however, clients are able to bring their lunch.
Water temperature in bathrooms was tested and measured within the required 105° F – 120° F. All the cleaning supplies are stored in a locked hall closet. The outdoor activity space and passageway are free of obstruction and hazards. There are no pools or large bodies of water.
