Complaint Investigation Report
The investigation revealed the following: Allegation#1- Staff abandoned residents.
The details of the complaint allege that facility staff left clients unattended on July 2, 2026. It was reported that a staff member was not relieved from their work shift and was planning on leaving the clients unattended. On 7/09/2026, from 2:00pm-4:00pm, the department interviewed staff (S1-S4) and attempted to interview clients (C1-C4). Due to cognitive difficulties the clients were not able to participate fully in the interview process. 4 of 4 staff denied the allegation Staff abandoned residents . All staff stated that they have never left the clients without supervision. Staff stated that there was coverage to take care of the clients on July 2, 2026, and no one left them alone.
The department reviewed the staff schedule for July 1, 2026, through July 2, 2026, and observed that a staff member was scheduled for each shift. The department interviewed staff members that were scheduled and found that each person worked there shift and the clients were not left alone. The department also reviewed a case management visit conducted on July 2, 2026, by Community Care Licensing Division, and observed that the department responded to a call that clients were left alone. The department came to the facility and conducted a health and safety check and found that staff were available as well as four clients in care. The department did not observe any immediate health and safety issues on the visit.
Based on interviews and records reviewed, there is insufficient evidence to support the allegation that Staff abandoned residents . Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated .
No deficiencies were found and no citations were issued for this complaint investigation.
An exit interview was conducted with Karen Chavez, Administrator, and a hard copy of this Complaint Investigation Report was provided.
