Facility Evaluation Report
On 01/10/24, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator John Paul Adeleye as the purpose of the visit was explained. The facility is licensed to serve 3 developmentally disabled adults ages 18-59 of which 1 may be non-ambulatory. Current census is 2 clients, clients are linked to the South Central Los Angeles Regional Center.
The facility is a single-story structure located in a residential neighborhood and consists of the following: (3) client bedrooms, 1 and a half bathroom, living room, kitchen, dining area, attached garage with washer and dryer, storage area ,emergency food and water supply and a shaded patio area. All facility rooms are clean and in good repair. LPA observed the following during inspection of client rooms: mattresses and box springs in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. LPA observed fully stocked bedding and towel closet. LPA observed bathrooms were found to be within Title 22 regulation. All bathroom fixtures are clean, in good repair, and working properly. LPA observed sufficient bedding, linens, and toiletries are accessible to clients. The water temperature properly measured between 105-120 F.. 2 fire extinguishes were observed to be charge, last fire drill conducted on 01/05/24. Carbon monoxide and smoke detectors observed and are operational. No weapons nor bodies of water on the premises. A supply of perishable and non-perishable food was observed. Landline and internet were observed. Facility has a surety bond and facility fees are current.
LPA conducted a records review of 3 staff records, 2 client records, and 2 medication administration record. No discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked.
During today’s visit no discrepancies were observed, exit interview conducted, and a copy of this report was provided.
