The inspector’s account

On 10/9/2025 at 2:00pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced required inspection visit to Bruce & Nelson Residential Facility. LPA was met by Kendra Nelson, Administrator, and the purpose of today’s visit was explained. The facility is licensed to serve four (4) developmentally disabled clients (age 18-59) of which four (4) may be ambulatory. Currently, the home has (2) ambulatory clients. The clients are South-Central Los Angeles Regional Center clients. None of the clients have Restricted Health Care Conditions and none utilizes postural supports or protective devices. The facility’s annual fees are due on 10/22/25 for $454.00.

The facility is a single-story building located in a residential neighborhood. It consists of the following: 3 bedrooms, 3 bathrooms, dining room, kitchen, living room, indoor and outdoor activity area, laundry room and an attached garage.

LPA conducted a review of (2) client records, (3) staff records, (2) clients Personal & Incidental records and reviewed the facility disaster plan. Staff records were complete with required documents and training. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (2) client medication administration records and medication and did not observe any discrepancies at the time of visit.

At 2:30pm, LPA and staff toured the facility. There are no bodies of water or firearm/ammunition on the premises. All client rooms were checked.
