Facility Evaluation Report

On Thursday, April 10th, 2025 at 11:09am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Marlene Wilson (Administrator) and the purpose of today’s visit was explained. The facility is licensed to operate for three (3) ambulatory, adults ages 18 through 59 with developmental disabilities. Currently, there are zero (0) residents residing at the facility. The facility fees is a balance of $454 due on April 29, 2025. LPA provided pin# 979710 with the option for the facility to make the payment online.

The facility is a two (2) story home located in a residential neighborhood. The property consists of the following: (downstairs) - a living room, a kitchen, a dining area (1) in staff personal living area with a single bedroom and personal bathroom attached with a washer and dryer set up, an attached garage with emergency essentials and a backyard with an outdoor shaded area and shed. (Upstairs) - (2) client bedrooms and one (1) shared bathroom.

Between the hours of 11:25 am - 11:50am LPA conducted a records review of (1) staff records, (0) client records, (0) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (0) Client Medication Administration Records as no residents currently resident at the facility and did not observed any discrepancies at the time of visit.
