Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced required annual visit today. LPA met with Yvett Sanchez, Case Manager and the reason for the visit was explained. Sanchez assisted LPA with the tour of the facility. Karina De La Rosa, Program Director, arrived thereafter.
The facility is licensed to serve 54 ambulatory and 6 non-ambulatory adults. The facility is operating within the scope of its license. The single-story unit is located within a business plaza in an industrial area of City of Industry.
The facility consists of the following: main entrance/reception area, conference room, client locker area, five (5) staff offices, four (4) restrooms, staff lounge, Library, Media Room, Quiet Room, Computer Room, copy machine area, Computer Room, Exercise Room, Art Room, Break Room, Janitor's closet and two (2) storage rooms.
LPA stoured the facility with Yvett Sanchez, Case Manager, and the following was observed:
A total of 12 clients were present at the facility during the visit. The staff to client ratio is 1:3 and ratios were adhered to during the visit. Clients were observed to be escorted from parking lot into the facility during arrival time and supervision by staff was observed. The facility is clean, safe, sanitary and in good repair. All passageways are free from obstruction.
