Facility Evaluation Report
On May 3, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Ana Kunz and Business Manager Dario Esguerra and explained the purpose of today's visit. The facility is licensed to operate for clients ages 18-59.
The facility is a two-story structure located in a commercial neighborhood. It consists of the following: (55) client's rooms and (55) restrooms, (2) activity rooms, (1) dining area, (1) commercial kitchen, (1) lobby (1) medication room (2) administrative offices and (1) courtyard patio area.
LPA toured the physical plant with Dario Esquerra. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in satisfactory condition, adequate lighting was provided, and storage for the client's personal belongings was observed. The client's rooms were inspected: 102, 103, 113, 115, 121, 122, 224, 225, 228, and 231. Call buttons were in working condition. Bathrooms were operational, with water temperatures measured at 110.7–118.0 °F. A comfortable temperature was maintained in the facility at 71 - 73 degrees F.
LPA observed the facility to be sanitary and furnished during the visit. Storage areas for personal hygiene items, cleaning supplies, toxins, and sharp objects were kept out of clients' reach. The kitchen was inspected, and sufficient perishable and non-perishable food was available and maintained properly. Fire extinguishers were charged, and smoke detectors and carbon monoxide detectors were operable. A review of the Fire Drill conducted on April 16, 2026 at 1:30 PM. The facility has a current Emergency and Disaster Plan reviewed and updated April 10, 2026.
