Facility Evaluation Report

On July 30, 2025, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced case management visit at the facility. Program Director Pat Ball welcomed the LPA. (LPA) Dabuet explained that the visit was related to a health and safety inspection conducted on July 2, 2025, as part of the case management process.

The investigation revealed that Client #1 (C1) was physically assaulted by Staff #1 (S1) on June 26, 2025. Witness #1 (W1) directly observed Staff #1 (S1) slap Client #1 (C1) on the hand/wrist. Additionally, Witness #1 (W1) and Witness #2 (W2) both saw (S1) shove and smack (C1) with the client's backpack, pushing it against (C1) 's stomach several times, which caused (C1) to fall to the ground. Furthermore, (W1 and W2) witness (S1) forcefully applied pressures on (C1) 's shoulders to keep (C1) seated on the bench. This behavior violated the client's rights and was highly inappropriate for a care staff member at the facility. Although (C1) did not appear to suffer serious injuries or show any marks afterward, actual harm is not necessary to validate the unwarranted actions. Two eyewitnesses corroborated the incident, a separate investigative from Harbor Regional Center and Moneta Learning Center provided further evidence of the inappropriate behavior.

According to the California Code of Regulations (Title 22, Division 6, Chapter 3), the following deficiency was observed, and a citation has been issued (ref. LIC 809-D).

An exit interview was conducted, and a copy of the Evaluation Report and Appeal Rights was provided to Program Director Pat Ball.
